I, the undersigned owner or authorized agent of the owner, have requested and accept the boarding, care, and services as provided for my pet(s) by Stay & Play Pet Resort, LLC (SPPR) and/or Baker Animal Clinic, PLLC (BAC) located in Stillwater, OK, 74075. I understand that while my pet(s) is in the care of SPPR and/or BAC that there is a risk of injury or illness or other damage to my pet(s).

In consideration for the acceptance of my pet's/pets' care and services provided by SPPR and/or BAC, I hereby knowingly and voluntarily waive any and all rights, claims, or causes of litigation or any action whatsoever arising out of my participation in any activity or services, professional or otherwise, provided by SPPR and/or BAC and do now hereby release and forever discharge SPPR and/or BAC, their respective past, present, or future officers, directors, shareholders, managers, members, affiliates, creditors, agents, attorneys, employees, contract staff, volunteers, heirs, representatives, predecessors, succesors in interest, assigns, insurance carriers, and any other person(s), firms, or corporations from any and all past, present, or future claims, demands, obligations, actions, rights, damages, costs, expenses and compensation of any nature whatsoever, whether based on tort, contract or other theory of recovery, and whether for general, compensatory, special, consequential, or punitive damages which any of the parties hereto, their agents, partners, owners, shareholders, investors, insurance carriers now have or may hereafter acquire on account of or in any way growing out of the acts or omissions which are the subject matter of the services provided by SPPR and/or BAC including, without limitation, medical emergency, illness, infectious or non-infectious diseases, bodily injury, loss of pet or property, damages, death of pet(s) and any economical or emotional loss that Owner may suffer as a direct result of their pet(s) participation in activities and services provided by SPPR and/or BAC, including transport of pet(s) to and from SPPR and BAC.

The owner, or authorized agent of the owner, agree to be financially responsible for any and all costs incurred as a result of any veterinary medical services required by my pet(s) for whatever cause arising from my pet(s) care at SPPR and/or BAC.
